Individuals wishing to use online survey tools such as Qualtrics, must complete the survey request form.
Some surveys will require approval of the research protocol by the Institutional Review Board for the protection of human subjects (IRB). The survey request form will be used to identify which surveys require approval. Approved use of the online survey tools is based upon the survey's contribution to the university strategic plan and IRB approval, if required.
Qualtrics is a Web-based survey instrument that allows the campus community to easily create surveys, collect and store data and produce reports.

Qualtrics Survey University has numerous resources including online tutorials, research articles and information about survey design. They also have over 300 sample surveys that can be used “out of the box” or questions can be copied and pasted into a custom survey created by you.

Go to http://www.qualtrics.com/uwrf.html
and create an account by providing your name and university e-mail address. Use Access Code Ejt9H. From the My Surveys page, click on the Create Survey tab and choose the Quick Survey Builder option (Recommended).
