If you are injured on campus and are not an employee (i.e. student or visitor) please get the medical attention you need and then fill out a "General Accident Report
" form. Please submit this form to Connie Smith, Risk Manager, 25H North Hall. Phone: 715-425-3344. (Note: If students are injured and do not need an ambulance but do need transportation to the clinic, Taxi Service is available to the student free of charge by calling 715-425-7878.) UWRF does not provide any type of compensation for injuries that occur on campus. Each student and visitor is expected to have their own health insurance to cover personal medical costs.
If you are an employee who is injured on campus, please inform your supervisor and get the medical attention you need. Then complete an "Employee's Work Injury and Illness Report". Your supervisor must complete the "Supervisor's Accident Analysis Prevention Report". Both of these forms should be submitted to Tracey Nyeggen in Human Resources, 218 North Hall.
