Instructions for Completing the Form

Note: This is NOT an order form for a new text.  This is to request books that Textbook Services already owns that should be issued to the students in your classes.  Please include if you would like iClickers to be issued. 

To be completed each semester that you teach:

• Summer 2013 due date: April 15, 2013
  
• Fall 2013 due date:  August 1, 2013
  

Please complete the form below. If you will not require any texts, so indicate in the comments section, and submit the form anyway. The information you provide is necessary to insure an accurate textbook issue, so please take a few minutes to submit the form below. 

If your classes use many books or if you prefer to not use the form and you would rather just send us an e-mail listing them out, that is fine.

Please place order for new texts using the Textbook Rental Order Form. Also please list newly ordered texts on the form below.

You can request up to five titles on this form. For additional titles, please submit an additional form.
