I-9 Employment Eligibility Verification Form
If you are a citizen of the United States, verification must be presented in person within three days of beginning employment at the University. You will need to go to Human Resources 218 North Hall, or present documentation during your benefit orientation. You may print out the I-9 Form and bring it with you. A list of acceptable verification documents are listed in the instructions of the I-9.
If you are a non-citizen, you should immediately contact Wendy Sader, Human Resources Manager, (715) 425-3926 to begin the process of securing proper work authorization from INS. Ms. Sader is located in 218 North Hall. |
Payroll Data Information Form
Employee information is used for entry into the UW-River Falls Payroll system and eSIS (UW-River Falls student information system). After entry into eSIS, employee computer access is granted. |

Faculty/Staff Network Account Setup
Activate Falcon Account which generates username and password that provides resources to access eSIS, computer lab login, email, homes, courses and web area on FalconFS fileserver. Once the Falcon Account has been activated, your supervisor or program assistant will be able to request additional accounts if needed.

Statement of Minnesota Residency
Persons of who are legal residents of Minnesota may use this form to:
1. Be exempt from the withholding Wisconsin income taxes from wages, or
2. Claim a refund of Wisconsin income taxes previously withheld. |
